If you want to know why Q has no clothes on it is because they were playing "bull ride" and she was told that she couldn't wear her princess dress to ride a bull. So, she took her dress off and obviously wasn't wearing anything underneath it!
I don't know if they really realize that they are cousins and not siblings, but that's not the worst thing that can happen. They really do love each other.
